Mission Statement and Core Values
Angel's Touch Home Care is a home health business providing skilled evaluations by a team of healthcare professionals to the patient in their home. Angel's Touch plans to provide the highest quality care to the patients we serve. Our approach is patient centered; striving each visit to help our clients attain their health care goals and remain as independent as possible within the home setting.


Arranging and Receiving Services
All home care services require a referral from your doctor. At this time, arrangements are made for a Registered Nurse to make an initial home visit to evaluate the patients needs, determine eligibility for home care services as well as the ability of the agency to meet the patient's needs in their home environment. If appropriate for home care, a plan of care is established with the treating physician and patient in order to meet the individual's health care goals. A nurse will be available 24 hours a day, 7 days a week. Patients and families can contact us during our regular business hours, Monday through Friday, 8:00 am - 4:30 pm. An on-call nurse is available to assist during non-business hours. Careful discharge planning is initiated at the time of your first visit. This ensures appropriate services are arranged, if needed, for aftercare.


Payment
Insurance can help pay the cost of home care. We accept most insurances including Medicare, Medical Assistance, and commercial insurances. Please check with your insurance company for home care benefits. If you like, we can call for you. We Comply with all State and Federal Regulations. Care is provided without regard to race, sex, religion, handicap, age, or national origin.


Services Available
Skilled Nursing - provide skilled assessments, wound care foley catheter changes, prepare and administer injectable medications and patient education on their disease process, diet, medication regime, and safety. Physical Therapy - provides skilled assessments, gait training, transfer training, and range of motion exercises; provide a home exercise program. Occupational Therapy - provide ADL/IADL training such as meal preparation, bathing, dressing skills. The patient is evaluated for home medical and adaptive equipment. A home exercise program is also provided. Speech Therapy - provides aural rehabilitation, voice and speech disorders evaluation and treatments. Aide - assists with personal care and bathing, dressing; and may take vital signs. Social Services - provide assessment of social, emotional, and financial factors impeding patients progress; assist in finding additional community programs needed to promote well being. Dietary Consultation - evaluate your dietary needs in relation to diagnosis; provides nutritional instruction.
